In order to book an appointment with the Cannock Chase Phlebotomy Service please use the following link:

You will need your NHS number and a mobile phone/e-mail number to book your appointment. Please select 'routine test' from the chosen options on the online form. Once you have booked your appointment you will receive a text message or e-mail to confirm. Please ensure that you bring the text message/e-mail confirmation to your appointment.

On the day of your appointment, please attend the Practice to collect your form. Please arrive 15 minutes prior to your scheduled appointment time. Please be aware that the Practice is closed on a Tuesday afternoon each month from 1pm on the dates below. If your appointment time is scheduled after this time, then please collect your blood form before 1pm on these dates:
